Renal lunacy and cardiac dysfunction in a homeless patient

The patient is a 39 year-old divorced white female who is homeless for the past several years. She has 2 children, ages 16 years and 10, not with her. The patient is suffering a bout with insomnia (again) due to “renal lunacy”–over self-taking her pain medication which can potentially be harmful to kidneys–and in her case, is, even at normal dosing. This causes her insomnia and her kidneys to over-produce at night, as well as increased nausea and hopefully-not-vomiting, all the while driving her eventually crazy like a stark raving mad lunatic. Thus, renal lunacy.

After giving up on trying to fall asleep, said patient proceeded to drink Mountain Dew. This highly addicting and tasty caffeinated beverage caused PVC’s to run rampant and thump in her chest, causing even more suffering and annoyance. Perturbed by this unsettling unease on top of a previous mood, she opted to “ride it out” rather than take the Inderal prescribed to her prn for such occasions. Patient did not self-check a pulse or BP, nor any other vital signs. For certainty, patient is still alive and somewhat well (at least physically, despite some PVC’s still) at the writing of this letter. Psychosocial status is yet to be determined.
